PLEASE NOTE
Form MUST be returned on Monday with student completed and signed by Parent/Guardian
Over 18s MUST sign the form themselves.
Students will NOT be permitted to enter school without this form completed and signed.
A second separate online form MUST also be completed. This will be sent to parents tomorrow via text message, this also must be completed before student returns to school but does not need to be printed and brought with student to school.
Please note if you are unable to print the form, you must collect from the school reception area either today or tomorrow. You do not need to contact the office just come in and take a sheet from the box. School open from 9am to 4pm both days
